7. e-mail address, contact phone numbers of the parents (legal representatives) of the child;
8. on the choice of the language of education, the native language from among the languages ​​of the peoples of the Russian Federation, including the Russian language as a native language;
9. on the need to educate a child under a special program or a disabled child under a rehabilitation program for a disabled child;
10. about the orientation of the preschool group;
11. about the necessary mode of stay of the child;
12. about the desired date of admission to training.

Additionally, the following documents may be required:

1. a document confirming the establishment of guardianship;
2. document of the psychological-medical-pedagogical commission;
3. a document confirming the need for training in a health-improving group;
4. Parents-foreign citizens present documents confirming the legality of the representation of the rights of the child, as well as a document confirming the applicant’s right to stay in the Russian Federation (with translation into Russian).

It is not allowed to require other documents for the admission of children to kindergartens.

After enrolling in a kindergarten, a personal file is opened for each child, in which all submitted documents are stored.
